What are Individual Day Support Services?
Individualized Day Support Services provide one-to-one opportunities so people with disabilities can maintain essential life skills and access preferred activities virtually and in the community. Examples include mobility safety, skill development, decision making, problem solving, self-care, ADL support, and more.
By directing your service, you decide what to explore and learn. Through undivided, personalized support, we begin by getting to know you while sharing our knowledge of available options. Together, we come up with an actionable plan and then provide your desired level of support.
Who is eligible to receive this service?
This service is for people who are 18 years of age or older and are diagnosed with a disability or a related condition. The person served can live in their own home, their family’s home, or in a group home and support is based on the individual’s plan and goals.
How can I pay for this service?
Lifeworks accepts Medicaid waiver dollars, county funds, and private pay.
Where is this service available?
Individual Day Services are available in the following counties: Anoka, Blue Earth, Brown, Carver, Dakota, Hennepin, Le Sueur, Nicollet, Ramsey, Scott, Waseca, Watonwan, and Washington.
