What is the Consumer Support Grant program?
The Consumer Support Grant program is authorized funding that covers staffing, goods, and services for people who want to manage their own support. Participants decide how to spend their budgets and can employ people they know such as family and friends as their support staff. The participant is responsible for finding, hiring, training, and scheduling their own staff and securing any additional supports such as assistive technology and home modifications.
Am I eligible to receive this service?
The Consumer Support Grant program requires that a person is authorized for Medical Assistance and lives in a typical home as well as needs ongoing support for daily activities.

Ann Ewert
Ann Ewert has worked at Lifeworks for more than a decade. She first began her career with us as a service facilitator before joining our fiscal support team in 2010. Ann provides support to individuals and families utilizing the Consumer Directed Community Supports (CDCS) and Consumer Support Grant (CSG) programs and is a Qualified Professional for the PCA Choice program. Outside of Lifeworks, Ann likes to spend time with her family and loves to be outdoors.

Lynae Greenebaum formerly worked as a service facilitator at Lifeworks Burnsville (now Lifeworks Apple Valley). She continued her career in social services working with children, families, and adults. She returned to Lifeworks in August 2016 and now serves as a service coordinator for Fiscal Support.
Lynae is passionate about working with families and assisting them in getting the services that they need. She enjoys meeting and getting to know the families that we serve.
In her free time, she enjoys running and has completed a half marathon, 15K, 10K, and multiple 5Ks. She enjoys watching and going to sporting events and spends a lot of her time watching her children play sports and just being with her family.

After joining Lifeworks, Cindy Mooers quickly demonstrated her strong sense of commitment and hard work. She is excited to help connect clients to resources that will enhance their lives. In her free time, Cindy likes to camp and hike, especially in Minnesota State Parks. She has camped and canoed in the Boundary Waters Canoe Area Wilderness three times, and thoroughly enjoys spending time in nature.

Before coming to Lifeworks, Emily was a program coordinator at REM Hennepin where she worked with people with disabilities for six years.
When she isn’t working, Emily enjoys watching and going to sporting events and spending time with family.
